Notifying the Public of Rights Under Title VI
Center for Developmentally Disabled posts Title VI notices on our agency’s website, in public areas of our agency, in our board room, and on our buses and/or paratransit vehicles.
Center for Developmentally Disabled operates its programs and services without regard to race, color, or national origin, in accordance with Title VI of the Civil Rights Act of 1964.

If you believe you have been discriminated against on the basis of race, color, or national origin by Center for Developmentally Disabled, you may file a Title VI complaint by completing, signing, and submitting the agency’s Title VI Complaint Form.

Complaints must be filed within 180 days following the date of the alleged discriminatory occurrence, and should contain as much detailed information about the alleged discrimination as possible.

Americans with Disabilities Act Grievance Procedure
This grievance procedure is established to meet the requirements of the ADA. It may be used by anyone who wishes to file a complaint alleging discrimination on the basis of disability in the provision of services, activities, programs, or benefits by the Center for Developmentally Disabled.
The complaint should be in writing and contain information about the alleged discrimination such as name, address, phone number, email address of complainant and location, date, and description of the problem. Alternative means of filing complaints, such as personal interviews or a tape recording of the complaint, will be made available for persons with disabilities upon request.
The complaint should be submitted as soon as possible, but no more than 180 calendar days of the alleged violation to:

Within 15 calendar days after receipt of the complaint, Carmen Jordan, Chief Human Resources Officer, will meet with the complainant to discuss the complaint and the possible resolutions. Within 15 calendar days of the meeting, Carmen Jordan will respond in writing, and where appropriate, in a format that is accessible to the complainant, such as large print, Braille, or audio tape. The response will explain the position of the Center for Developmentally Disabled and offer options for substantive resolution of the complaint.
If the response by Carmen Jordan does not satisfactorily resolve the issue, the complainant may appeal the decision within 15 calendar days after receipt of the response to the CEO, Sarah Mudd or designee. Within 15 calendar days after receipt of the appeal, the CEO, Sarah Mudd or designee will meet with the complainant to discuss the complaint and possible resolutions. Within 15 calendar days after the meeting, the CEO, Sarah Mudd or designee will respond in writing, and, where appropriate, in a format that is accessible to the complainant, with a final resolution of the complaint.
